COVID-19 Vaccination Update January 13, 2021
Beginning today, the Santa Clara County Health and Hospital System will be offering COVID-19 vaccinations to Santa Clara County residents who are 75 years of age or older. Any local resident in this age group who is not a regular patient of Kaiser, Palo Alto Medical Foundation, or Stanford Healthcare can now schedule an appointment through one of Santa Clara Valley Medical Center’s (SCVMC’s) vaccination clinic sites by visiting www.sccfreevax.org and clicking on the “schedule a vaccination appointment” link for SCVMC. Appointments are required for COVID-19 vaccination (no walk-ins are accepted).

In our clinic, you will have direct access to Dr. Nalawadi and her staff, either in person or through our online portal or over the phone. We understand the challenges of getting appointments for those working full time. We will have non-urgent appointments and physicals on
Two Saturdays a month 9am - 12noon and
Wednesdays 2pm - 7pm.
These appointments are only available when booked in advance through our staff.
In order to avoid crowding, when coming in for your in person appointment, call from our car park and wait until you are invited to enter clinic reception area

We accept Aetna, Anthem Blue Cross, Blue Shield of California, Cigna, Healthnet, SCIPPA, Medicare, TriCare, and United Healthcare, just to name a few. Individual plan types vary by payer and employer group so it’s best to check participation with your carrier before booking an appointment.

Smruti Nalawadi M.D.
Smruti Nalawadi, MD, is an internal medicine physician with over 19 years of experience, having served the Morgan Hill community for last 4 years. Dr. Nalawadi has experience in treating complex comorbidities like diabetes, hypertension, metabolic syndrome, obesity, women’s health, heart disease, lung disease, nutritional problems, stroke, and complex geriatric conditions.
Sarina is a medical assistant here at BASS Medical group working under the supervision of Dr. Nalawadi.
Seena Khajavi is a medical assistant here at BASS Medical group working under the supervision of Dr. Nalawadi.He has completed his Back Office Medical Assistant Certification at Silicon Valley Adult Education (SVAE), and is currently a full time student at San Jose state University studying Biology with a concentration in Systems Physiology. Seena has plans to apply for graduate programs in Nursing and physicians assistance. In his free time he enjoys reading, playing guitar, mixed martial arts, badminton, and cooking.
